Benefit Details:

1. Milestone 60 Benefit

Amount: Php 500,000.00

This benefit is an endowment insurance plan that provides for both maturity and death benefits. The maturity benefit is the amount payable to the Owner of the Policy upon the survival of the Insured at the maturity date while death benefit is the amount payable to the designated Beneficiaries upon the death of the Insured.


2. Dread Disease Benefit

Amount: Php 1,500,000.00

This benefit provides for a lump-sum amount payable to the Insured upon contraction of a covered Dread Disease more than 90 days after the Policy Effectivity Date or from the date of last reinstatement, and such diagnosis occurred prior to the Policy Anniversary before the Insured's 61st birthday.

3. Accidental Death & Dismemberment or Loss of Use Benefit

Amount: Php 1,500,000.00

This benefit provides for Accidental Death, Dismemberment, or Loss of Use and is payable when the loss incurred has resulted directly and solely from a bodily injury sustained within 180 days from an accidental event.



4. Bereavement Assistance Benefit

Amount: Php 125,000.00

This benefit provides for an additional amount payable to the designated Beneficiaries upon the death of the Insured.



5. Accidental Total and Permanent Disability Benefit

Amount: Php 1,500,000.00

This benefit provides a lump-sum amount payable to the Insured in the event of the Insured's total and permanent disability due to an accident within 180 days from the date of the accidental event.



6. Accidental Medical Expense Reimbursement Benefit (Deductible: 500)


Amount: Php 125,000.00

This benefit provides reimbursement benefit in the event that the Insured shall require treatment bya  physician or use of hospital facilities, whether in patient our outpatient, by reason of accidental bodily injury, and commencing within 30 days after the date of the accident. The amount payable to the Insured is the actual, reasonable, customary, and necessary medical expenses incurred for such treatment, hospital charges, and nurse's fees which exceed the deductible of Php 500 up to the benefit amount.



7. Daily Hospital Income Benefit

Amount: Php 2,500.00 per day

This benefit provides income for each day during which the Insured shall require treatment by a physician and require hospital confinement due to an accidental bodily injury or illness in accordance to the following:

- Daily Hospital Income Benefit (DHIB) shall commence from the 1st day of hospital confinement if due to accidental bodily injury while benefit shall commence from the 3rd day of the hospital confinement if due to illness.

- Daily Hospital Income Benefit (DHIB) shall not exceed 30 days per accident or illness, up to a maximum of 60 days per policy year, and 180 days during the lifetime of the Insured.

- Daily Hospital Income Benefit (DHIB) due to preexisting conditions shall only be provided after the first 9 months from the Policy Effectivity Date or date of last reinstatement, provided there is no failure to disclose, misrepresent, or conceal material information in the original application or reinstatement.

 - An additional DHIB is provided if the Insured is confined in an Intensive Care Unit (ICU) or due to an illness classified as a Dread Disease. In case the illness for which the Insured is confined in the ICU is classified as a Dread Disease, only one additional DHIB is provided.

Surgical Expense Benefit is provided in the event that a surgical operation named in the Schedule of Surgical Operations is performed on the Insured by a surgeon. The amount payable to the Insured on each covered operation shall not exceed the Percentage of Maximum Surgical Benefit as stated in the schedule. The Maximum Surgical Benefit is equal to twenty (20) times the Daily Hospital Income Benefit.

Convalescence Benefit is provided in the event that the Insured is continuously confined in a hospital for 30 days or more. This benefit is equal to twenty (20) times the Daily Hospital Income Benefit and shall only be payable once per illness.
